It seems pretty unlikely that all those items would be faulty. You can get a TPS fault code quite easily if it's been removed from the throttle body and not put on in exactly the right position. In fact, if you just turn the switch on the ECU without fully depressing the accelerator and releasing it at the right time (varies between years of manufacture) you will always get a TPS error code, as it needs to see fully closed and WOT to register as ok. Probably the same situation with the ignition code if the key isn't on at the right time.

I had this problem on the Laurel for a while, always getting TPS and CAS fault codes, until I found the correct test procedure - most of the online guides don't mention when to do this stuff. Hope this helps.

Does it make any difference to how it is running if you unplug the AFM and try start it up? Because with out an AFM plugged in the car will run incredibly rich almost to the point of it not running but it should still run which it seemed thats how it was running on the weekend. If it runs the same without the AFM then i would suggest you start with figuring out the AFM wiring.

Also the AFM gets a 5V feed from the ECU. Get a volt meter and turn the key on and find the 5V wire and make sure it is at a steady 5V then leave the volt meter all hooked up and start the car and make sure there is always a constant 5V there as if there is not then it wont be getting a proper reading from the AFM. In which case i suggest you re check all your earths for the ECU/Engine and make sure you have a 12V+ direct from the battery for your ECU power as ECUs will run like shit if they dont have a dedicated 12V feed for themselves from the battery. If that doesnt help then you will need a new ECU as the voltage regulator circuitry to do the step down from 12V to 5V will be fried.

Hope that helps.
